Example: Age 35, Non-smoker, $50,000 = $20.91 per month. (All rates listed include Waiver of Premium (PW) and Accidental Death Benefit (ADB)

2. There are two additional forms that need to be signed. They are listed as NAIC form and LBR form. Be sure to send these signed forms with your application and Election to Participate Agreement to Provess. Failure to include these will delay the issuance of your policy.
